Usual Intake Questionnaire


Describe a normal day of eating/drinking for you, beginning with when you wake up through the time you go to sleep. In addition to your consumption details, include any explanation as to why you chose to eat at these given times. Be as specific as possible, and include what you drink throughout the day too.


Examples:

  1. Time - Morning: I don't have time to eat a big breakfast in the morning, so I usually just have coffee or tea and a banana.

  2. Time - Afternoon: I eat a snack right before noon, usually a piece of fruit (apple/banana) and a chocolate protein shake because I usually workout during my work lunch break.

  3. Time - Bedtime: I am normally hungry before bed, so I usually eat a bowl of cereal like cheerios with 2% milk in the evening.

Things to consider:

  • Be as specific as possible.

  • Do you eat breakfast?

  • Do you intermittent fast?

  • What snacks do you eat throughout the day?

  • Don’t forget to include the beverages you consume, including water.
